Each Family Connection collaborative engages in an ongoing planning process. Local citizens, agencies, and community organizations have an opportunity to establish a shared vision for children and families and chart a course to get there.

Each collaborative develops a three-year community strategic plan, based upon a community assessment. This plan details goals, benchmark objectives, and strategies to improve results for children and families and facilitate collaborative development and systems change. In order to implement the strategic plan year by year, the collaborative develops an annual plan, with an annual results evaluation plan, that defines how the collaborative intends to make and measure progress toward its long-term goals and benchmark objectives. Each collaborative submits all plans to the Partnership for approval.

Community Assessment:

A community assessment involves community leaders, service providers, and residents in a process that documents information on community strengths, needs, challenges and opportunities, promotes dialogue and builds consensus for community strategic planning.

Strategy Development:

Effective strategies come from evidence-based research and are comprehensive, prevention-oriented, relevant to the desired outcomes, family-centered, and help reduce risk factors and strengthen protective factors. The three-year community strategic plan describes how a strategy will contribute to achieving the desired benchmark objectives by providing supporting evidence. FCP has developed resources to assist with strategy development.

Evaluation Plan:

The annual results evaluation plan assesses changes in individuals, families, and/or the community resulting from the strategy being evaluated. The plan describes when and how the findings are to be reported and used to inform decision-making.
